His contract is fully guaranteed.
Trey Lance 5th-Year Option
Lance (and the rest of the 2021 first-round picks) can have their fifth-year options picked up until May 1, 2024.
All drafted rookies are given four-year contracts, and teams can extend that contract to a fifth year with players selected in the first round.

Trey Lance Extension
Lance is eligible for an extension after the third year of his rookie contract, which is after the upcoming 2023 NFL season.
Whether or not that extension will ever manifest is more complicated than most rookie quarterback scenarios. The 49ers spent a lot of draft capital to acquire Lance, but he’s also started a total of five football games in the last three calendar years.
Oh, and one of those five was a college football game against Central Arkansas in the strange 2020 FCS football season, which was dramatically reshaped by COVID.
For a quarterback who was widely labeled as a high-ceiling project, his experience playing football beyond the high school level just isn’t there, perhaps more than any other first-round player in modern NFL history.
The 49ers may see the future of their franchise in him; they may also see an asset to be moved as soon as possible.
The NFL world will have to wait and see whether the team prefers Brock Purdy’s contract over Lance.
